Twenty-five patients with CD and actively draining perianal fistulas were randomized to receive ciprofloxacin mg, metronidazole mg, or placebo twice daily for 10 weeks. The primary endpoint was remission at 10 weeks. Results: Ten patients were randomized to ciprofloxacin, 7 to metronidazole, and 8 to placebo. No serious adverse events occurred. Conclusion: Remission and response occurred more frequently in patients treated with ciprofloxacin but the differences were not significant in this pilot study. Ciprofloxacin was well tolerated. A single 1-g dose of oral azithromycin is as effective as a seven-day course of oral doxycycline, mg twice a day, for the treatment of chlamydial infection. Erythromycin and ofloxacin are alternative agents. Four single-dose therapies are now recommended for the management of uncomplicated gonococcal infections, including mg of cefixime, mg of ciprofloxacin, mg of ceftriaxone or mg of ofloxacin. Advances in the treatment of bacterial vaginosis also have been made.

Despite the broad antibacterial spectrum of ciprofloxacin, most anaerobic organisms are resistant to the drug, whereas several gram-positive organisms are only moderately susceptible. Thus, in some clinical situations, combined treatment with ciprofloxacin and metronidazole or clindamycin could be useful. Ciprofloxacin mg was administered alone and in combination with clindamycin mg or metronidazole mg ; all drugs were given intravenously over 30 minutes. Blood samples for determination of serum bactericidal activity against five different aerobic and two anaerobic bacterial species a total of 58 strains were obtained one hour and six hours after drug infusion. Abstract Despite the broad antibacterial spectrum of ciprofloxacin, most anaerobic organisms are resistant to the drug, whereas several gram-positive organisms are only moderately susceptible.
